We work together to ensure the processes are in place for improving the health of our communities.

Each of the 12 Community Advisory Councils (CACs) partner with their local public health authority, local mental health authority, hospital systems and community leaders to develop a shared Community Health Assessment (CHA) process and assess developing Community Health Improvement Plans (CHIPs).

The EOCCO Regional Community Health Improvement Plan

The Regional Community Health Improvement Plan (CHIP) was approved by the EOCCO Board of Directors on June 23, 2014. The regional CHIP represents all 12 counties (listed above) that are included in the Eastern Oregon CCO.
