The Oregon Health Authority (OHA) has a new website where members can complete an online survey to report their positive case. Members can also get information on isolation and quarantine plans, and ways to keep others safe while they recover.
Please visit, Oregon.gov/positivecovidtest to find the Case Investigation Survey. Members can also use the COVID-19 Case Support Hotline, 866-917-8881. They are open from 8 a.m. to 6 p.m. Monday through Friday, and 10 a.m. to 4 p.m. Saturday. Hotline staff can answer questions about isolation, quarantine, resources to help them during isolation and can give general health information.
