The coverage criteria for HCV medications is reviewed and updated on a periodic basis. We wanted to ensure you were aware of the updates that went into effect on March 1, 2019, most notably the removal of fibrosis score requirements. If you have patients that you believe may be candidates for treatment, please submit an authorization request to EOCCO via CoverMyMeds (www.covermymeds.com).
