EOCCO and Unite Us, the nation’s leading technology company connecting health and social care are partnering to expand Oregon’s coordinated care network, Connect Oregon, that addresses the unmet needs of people and families throughout the state. The expansion will initially include Baker, Gilliam, Morrow, Sherman, Umatilla, Union, Wallowa, and Wheeler Counties, with plans to expand to Grant, Harney, Lake, and Malheur Counties on July 19th, 2022.
Unite Us helps connect people to much-needed resources and services such as assistance with housing, food security, and benefits navigation. With the expansion to the EOCCO service area, Connect Oregon is becoming a truly statewide network to address unmet social needs.
EOCCO acknowledges the importance of addressing social care to improve a person’s overall well-being. By expanding Connect Oregon into the EOCCO service area, healthcare, dental, behavioral health, and social service providers will have access to a robust network of partners to streamline care coordination and expand access to resources.
Together EOCCO and Unite Us will be able to securely connect people in need with community-based resources that can be difficult to navigate. The network provides a central point of contact where health care providers, social service organizations, and individuals can securely access and refer people to needed services while monitoring progress and measuring outcomes.
