Become an HRSN provider
EOCCO is expanding its network of HRSN providers who are committed to offering services that are culturally sensitive, linguistically accessible, and trauma-informed. All providers must meet specific qualifications to ensure consistent, high-quality care.
Qualifications for becoming an HRSN service provider
To become an HRSN service provider, you must:
- Be accessible to HRSN participants
- Demonstrate your ability and/or experience with effectively serving at least one Oregon Health Authority (OHA) HRSN priority populations
- Provide culturally and linguistically appropriate, responsive and trauma-informed service delivery
- Demonstrate a history of responsible financial administration via recent annual financial reports, an externally conducted audit and/or other similar documentation
- Meet member needs for the activities listed in the benefit service descriptions, or can connect members to the appropriate service provider or vendor while participating in closed loop referral processes
Housing services providers
Housing services providers must have knowledge of principles, methods and procedures of housing services covered under the waiver, or comparable services meant to support individuals in obtaining and maintaining stable housing.
Nutrition services providers
Nutrition services providers must have knowledge of principles, methods and procedures of the nutrition services covered under the waiver, or comparable services meant to support an individual in obtaining food security and meeting their nutritional needs. Nutrition service providers must follow best practice guidelines and industry standards for food safety.
Outreach and engagement providers
HRSN outreach and engagement and benefit linkages providers must have knowledge of principles, methods and procedures of these services or comparable services meant to outreach to and engage the populations covered under the waiver and connect them to benefits and services to meet their needs.
EOCCO will work with all providers to assess their readiness and staffing to set referral capacities and ensure that providers are qualified to serve HRSN members. EOCCO can also provide HRSN training as well as other training and resources as it relates to trauma informed care, DEI, translation and interpreter access.
1. Enrolling with EOCCO
To become an HRSN provider, you must first enroll with EOCCO.
Existing EOCCO Providers: If you are already DMAP enrolled as a provider with EOCCO (with a non-HRSN provider type) you must go through the enrollment process again to become an HRSN Service Provider with the specialty codes listed above.
You will need to submit one application for each of the specialty types below. You can use the same application, just rename each version with the specialty and specialty code listed below.

Step 1
Fill out a new Rendering or Facility/clinic application. You must complete one application for each HRSN specialty type you intend to bill for (see table above).
- For example, if you plan to provide HRSN services in climate, housing, nutrition and outreach and engagement, you will need to submit four applications. Please rename each version with the specialty and specialty code
- A complete and signed Provider Enrollment Agreement (Form 3975) is required (included in the application)
- Include a copy of your current facility license and any applicable previous year licensees covering the service dates listed in the enrollment request
- Provide a current-year business W-9 form
Step 2
Email your completed application to ProviderDMAPApps@modahealth.com. Be sure to include “HRSN” in the subject line.
